Partnering for Better Health
BJC partners with employees to improve their health by offering programs and personal health-care information that contributes to better health.
Health promotion steps in 2010 continue to include covered spouses and same-gender domestic partners, if applicable. To participate in the Gold or Silver medical options, or to receive the medical premium discount in the Bronze or Resident POS options, the following steps must be completed . . .
Employees must:
- Accept a BJC health pledge during benefits enrollment
- Complete an online confidential BJC health risk assessment within 90 days of their eligibility date
- If they use tobacco, enroll in a tobacco cessation program within 90 days of their eligibility date
Their covered spouses or same-gender domestic partners must:
- Accept a BJC health pledge during benefits enrollment; employees who accept the health pledge are assumed as also acknowledging acceptance of the health pledge by their covered spouses or same-gender domestic partners
- Complete an online confidential BJC health risk assessment within 90 days of their eligibility date; the biometric information is optional, although the more thoroughly completed the health risk assessment, the more informative their confidential report -- which provides information on their health risks and how to help reduce them; learn your biometrics at a BJC Employee Health Fair
- If they use tobacco, enroll in a tobacco cessation program within 90 days of their eligibility date
Each employee and each spouse or same-gender domestic partner will receive a confirmation number when they have completed ALL requirements for the health risk assessment. Save your confirmation number, and keep it for your records. (This does not include the requirement to enroll in a smoking cessation program, if applicable.)

If you participate in a non-BJC HealthCare tobacco cessation program, you need proof of enrollment to receive consideration for the Gold or Silver medical options, or the medical premium discount in the Bronze or Resident POS options. The proof of enrollment form is in PDF format and requires Adobe Acrobat Reader. If you don't have this software, go to Adobe for a free download.

Get Your Biometrics
You can provide your biometric information when you complete the confidential BJC health risk assessment:
- Height without shoes in feet and inches
- Weight wearing light clothing, no shoes
- Hip measurement to the nearest inch (measure around widest part of hips)
- Waist measurement to the nearest inch (measure waist at navel)
- Systolic and diastolic blood pressure (mmHg)
- Total and HDL cholesterol (mg/dl)
- Blood glucose (mg/dL) (blood sugar)
- Answer the question of whether you fasted 9 or more hours before blood was drawn
Employees can obtain their biometrics during a BJC Employee Health Fair or an annual physical with their personal physician.
Personal health information is not shared with BJC -- it is confidential.

Your Health Measures
If you attended a Health Fair and have lost the card documenting your health measures, call 314.996.3721 and leave a message providing:
- Your name
- Your place of work
- The location of the Health Fair you attended
- The approximate date of the Health Fair you attended
- Your daytime telephone number
Calls will be returned between 10 a.m. and 2 p.m. Tuesdays and Thursdays.
The screening staff are the only ones with access to the health measurement data. They will request your birth date to verify your identity.
